Alzheimer and Related Disease Registry Purpose of Registry The purpose of the Registry is to develop a database of individuals with Alzheimer`s Disease and Related Dementias. Its goal is to enhance the ability of the Alzheimer Society of Newfoundland and Labrador to provide programs and services, to respond to the needs of those affected by the disease and to offer support to families and caregivers. Confidentially of Information Collected Strict confidentially policies and procedures have been implemented in order to ensure the protection of the registrant’s information. Two types of data will be collected:1) core data sets include name, date of birth, address and phone number, diagnosis and identification of caregiver’s relationship to the individual. 2) Supplementary data sets include programs and services used, socioeconomic data and impairments (vision, hearing, etc.) Enroll Today By participating in the Alzheimer and Related Disease Registry you will be helping many people diagnosed with Alzheimer`s Disease in Newfoundland and Labrador.
